Laser Doppler Imaging Opens New Perspectives for Intra-Operative Brain Mapping

Laser Doppler flowmetry (LDF) is a well-established technique to probe for perfusion of blood flow at a single measurement locus. Recent technological advances have allowed the principle of LDF to be extended to imaging (LDI); by these means a full perfusion map can be created at a rate of about 1-2Hz.

Accurate localization of functional brain regions is vital in neuroscience, both for fundamental and clinical research. We show the case study in neurosurgery where LDI is applied for intra-operative human brain mapping to avoid iatrogenic damage. The LDI device is integrated within the surgical microscope and used to observe the cortical surface from a distance of 15-20cm,and being a class 1 laser device is non-invasive. During recording, the patient performs a series of predetermined tasks. The spatio-temporal dataset is then analyzed using general-linear-model fitting and the wavelet transform for improved noise robustness. The active regions identified from the statistical maps are confirmed by electrocortical stimulation mapping (which is the golden standard during neuro-surgery), and also compared with pre-operative fMRI activation maps.
